Project Uplift
provides services to individuals who are affected by any of the following
problems: mental illness, substance abuse, acute or chronic financial or health
issues. Many individuals and families served by Project Uplift are at risk of
becoming homeless or are currently homeless when they initiate services through
our program. Its primary goal is to ensure that clients are living safely, have
an optimum quality of life and have the best opportunity to remain in the
community and avoid psychiatric hospitalizations.
Project Uplift is a
holistic program that addresses all critical life domains including basic needs
(food, clothing and shelter), and physical, mental and emotional health. Project
Uplift is a warm, client-centered program that values diversity in all ways. It
is integrated into the county-wide continuum of care for persons affected by
mental illness, substance abuse and/or financial challenges. As such, it works
closely with providers of psychiatric care, day treatment, vocational services,
housing providers, social services, hospitals, etc.

Three levels of case management are offered:
- Outreach/short-term crisis case management.
- Ongoing case management for persons affected by mental illness.
- Supportive case management for persons affected by mental illness and chemical addictions.

- referral from the single entry committee for case management and,
- referrals from other agencies, hospitals and organizations.
- mental health services for persons affected by HIV/AIDS
- advocacy to improve and coordinate the care of the homeless and those at risk of homelessness
- drop in supportive counseling services,
- limited emergency housing funds
- shelter/housing referrals
- emergency referral
- clothing bank referrals
- food referrals
